  • Infants
  • Postpartum women
  • Pregnant women

Income Guidelines

  • Amounts are based on income before taxes. 
  • Those with SNAP, TANF or AHCCCS automatically qualify. 
  • If you are pregnant, the unborn baby is counted as a family member.

How to Apply for WIC

Visit a clinic near you to apply (walk-ins are served after previously scheduled appointments).

When you visit, please bring the following items:

  • A form of identification - like a photo ID (driver's license, state ID), birth certificate, or immunization record
  • Proof of address - like a driver's license, state ID, or utility bill
  • Proof of income - like the last 30 days of pay stubs or letter from AHCCCS, SNAP, TANF, Section 8 housing
  • Each family member applying for WIC must be present
